While HA has been improving the experience of the Bluetooth functionality, there are some areas that may still be confusing because its settings are spread out in many places and many of the displays are hidden deeper.
Here's a simple demonstration, where a working Bluetooth Proxy is shown for HA, meaning that those changes can be seen.
Since Bluetooth Agents can work together or with the HA's own physical Bluetooth adapter, if necessary you can temporarily turn off some Bluetooth receiving devices in Bluetooth to focus on the ones you're interested in.
First of all, if the Bluetooth Proxy is properly connected to the HA, traces of it will be visible in the Bluetooth integration and the ESPHome integration.
If it's not working as expected here, consider reinstalling the firmware using our online tool and pairing the network.
Now, if the above step is correct, then you can start observing the Bluetooth signal it receives.
